personal statements

Hi, my name is Adam, and I just recieved my notary test results back with a passing grade *yay*, and I was looking into getting the required surety bond to finish my application process.
I have printed out the required documents, but on the personal statement form, I am unaware as to the information that is required to complete this form. Is there anyone that can help me, and answer my questions on this?

Unfortunatly, you are out of luck. JW Bond Consultants, Inc. does not write notary surety bonds. I will be glade to answer any specific questions you have on how to complete a personal financial statement if you let me know exactly what sections you need help with.
